Fix SMTP EHLO handling for authentication and TLS 
This patch fixes SMTP authentication when credentials come from the module configuration by loading them before choosing `HELO` or `EHLO`.

It also fixes the SMTP monitor's `STARTTLS` check and sends `EHLO` again after the TLS connection is established.

Jamie, I made this digging based on your latest change https://github.com/webmin/webmin/commit/65de483f3 to fix issue #2879.
