TOC not being generated when asciidoctor-pdf 2.0.x is installed #246

When asciidoctor-pdf 2.0.x is installed the TOC is not being generated in the pdf. Updated the Gemfile to install 1.6.2 until we can figure out what is causing the change. Will see if I can dig up logs to add to this issue.

Delete Branch "%!s()"

Deleting a branch is permanent. Although the deleted branch may continue to exist for a short time before it actually gets removed, it CANNOT be undone in most cases. Continue?
